Memorial/ Tribute Form



Lakeside Memorial and Tributes

 

Individuals can arrange memorials or tributes for families and friends through the Lakeside Chautauqua Foundation.  Families are encouraged to contact the Advancement office to notify about memorial and tribute gift designations at 419.798.5396 or email advancement@lakesideohio.com.  Please speak with Gretchen Colon or Lesley Roush. 

 

Sample Information for publishing:  “Memorials/ Tributes can be made to the Lakeside Chautauqua Foundation: 236 Walnut Avenue, Lakeside, Ohio 43440 in memory/ tribute of _________________________.  Please add a note in the check memo line specifying the memorial.”

 

Individuals donating to the designated memorial or tribute are encouraged to complete the Memorial/ Tribute form for the Lakeside Chautauqua Foundation so that we can properly notify the family.  Please download the above form.
